Builder Games – Teams Release Forms

I hereby accept any and all responsibility for, and assume the risk of any and all injury or damage to my person which might arise directly or indirectly as a result, and or participation in the Builder Games hosted by People’s Self-Help Housing.

I hereby expressly release, discharge and hold harmless from any liability whatsoever, People’s Self-Help Housing, expressly including but not limited to their employees, volunteers, limited partners, and Board of Directors, except for injuries caused intentionally or by willful misconduct. I certify that I am familiar with the contents of this release, that I have read and understand the same, and that it is my intention by signing this release that the same be binding not only on me, but my heirs, administrators, executors, successors, and assigns.

By signing this document I confirm I am 18 years or older.

I hereby give People’s Self-Help Housing Corporation, its subsidiaries and/or its representatives (herein collectively known as “PSHH”) the absolute right and permission to use my name, my image, my voice, and/or still or video photographs for publicity purposes and to make reproductions in any media, including but not limited to the Internet, social media, and to publish, and/or market such images at PSHH’s sole and absolute discretion of such images without compensation.

I agree that the right herein granted includes the right for PSHH to edit, reproduce, use and distribute the picture(s) in any manner stated above. PSHH shall be under no obligation to actually use the name, likeness or biographical information in any media.

The rights herein shall inure to the benefit of PSHH, its licensees, successors, and assigns. This release is subject to the laws of the State of California.

I have read and fully understand the meaning and effect of this release and, intending to be legally bound under this release, sign this release.
